This form is completed personally by the model - the person on whom the competition work is carried out. The contestant does not complete it on their behalf.
How it works
We collect only what is necessary
First name and surname, e-mail address, an indication of the entry and category, and a declaration of being of legal age. We do not ask for a national identification number, home address, telephone number, date of birth or a scan of an identity document.
The model must be of legal age - at least 18 years old.
Wording of the consent - version AE2026-M1 (English translation for information)
I consent to the free-of-charge recording, reproduction and publication by the Organiser of Amber Eyes of my image, including close-ups of my face and of individual parts of my body, recorded as part of the competition work carried out during Amber Eyes.
The consent covers use in: the documentation of the competition and the assessment of the work by the Jury, coverage of the event, on the ambereyes.pl website, on the Amber Eyes profiles and channels, and in materials promoting the event.
I acknowledge that photographic documentation is taken in three stages: before the treatment, immediately after the treatment and during the follow-up visit 12-18 days after the treatment, including en face shots, profile shots and shots with a natural smile.
I acknowledge that the materials are transferred to the Organiser through an external file transfer service whose provider may process data outside the European Economic Area on the terms described in the Privacy Policy, and are then made available to the Jury in anonymised form, without data identifying the contestant, for the purpose of assessing the competition work.
The consent is voluntary. I may withdraw it at any time, without giving a reason and free of charge, by writing to kontakt@ambereyes.pl. Withdrawal does not affect the lawfulness of use before the withdrawal.
